Job summary

Heartland Hospice in the United States is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes and facilities. You will collaborate with the daytime care team, triage urgent symptoms, and document visits in the EMR to ensure continuity of care.

Requirements include current IA RN license, 1+ year nursing experience, CPR, and reliable transportation.

Qualifications

• Current IA RN license or eligibility to obtain.
• Minimum 1 year of nursing experience, preferably hospice or home care.
• Strong clinical judgment and compassion in urgent care situations.
• Reliable transportation and valid driver's license.
• CPR certification is required.

Responsibilities

• Respond to patient needs after hours, triaging and addressing symptoms.
• Provide direct patient care and support in patients' homes or facilities.
• Coordinate with the interdisciplinary team to implement the care plan.
• Document visits and communications accurately in the EMR.
• Participate in quality improvement activities and training.
