Heartland Hospice seeks an After Hours RN to provide compassionate, patient-centered care during evenings, nights, weekends, and holidays. You will respond to urgent needs, coordinate with the care team, and document visits to support the Plan of Care.

You will work with LPNs/LVNs, CNAs, and other staff, educating families, and ensuring high-quality hospice services in patients’ homes. A state RN license and CPR certify you for this important role.

Qualifications

• Current RN license in the state of requested employment (or eligibility)
• 1+ years of nursing experience
• Compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes or other facilities
• Valid driver’s license and reliable transportation
• Current CPR certification

Responsabilités

• Respond to patient needs after normal business hours.
• Serve as primary liaison between patients, families, physicians, and hospice team.
• Document clinical visits and communications.
• Evaluate patient and family needs to guide care decisions.
• Educate patient, family, or caregivers about Plan of Care.
• Collaborate with Interdisciplinary Group to provide holistic care.
• Maintain and update medical records and Plans of Care.
