You will screen, document, and manage referrals while collaborating with care teams to ensure accurate census data and smooth billing processes. This role emphasizes compassionate communication, organization, and adherence to confidentiality standards.

Qualifications

• Excellent verbal and written communication skills.
• Experience in administrative or intake roles in healthcare preferred.
• Ability to maintain confidentiality and handle sensitive information.

Responsabilités

• Coordinate the admission process from first contact through intake.
• Screen clients by phone and in person to determine needs and eligibility.
• Maintain relationships with referral sources and clients, delivering excellent service.
• Manage referral activity and waiting lists per applicable standards.
• Complete admission documentation and enter demographic, financial, and consent data.
• Coordinate insurance and financial information with care management and billing.
• Maintain program census and support reporting requirements.
• Assist clinical staff with admin tasks, correspondence, forms, and docs.
• Conduct chart reviews and follow up on documentation deficiencies.
• Ensure timely data entry and reporting.
