Chapters Health System in Florida is seeking an Admission RN to assess patients in their residence or hospital and initiate care with our Care Navigation team. You will act as a liaison, ensuring smooth transitions into our services and representing the organization professionally.

The role requires an active RN license, at least one year of nursing experience (hospice/hospital preferred), and experience with EMR/EHR systems. A valid driver's license and reliable transportation are also needed.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems; strong assessment skills.

Responsabilités

• Complete clinical admissions for all services and explain offerings empathetically.
• Coordinate with physicians and care teams to initiate care plans.
• Educate patients/families on admission requirements and options.
• Obtain consents, orders, and initial assessments in compliance with policy.
• Facilitate safe discharge planning and timely follow-up communications.
