Job summary
Chapters Health System in Florida is seeking an Admission RN to assess patients in residence or in hospitals and initiate care within Chapters Health System.
Candidates must hold an RN license in Florida, have at least one year of nursing experience (hospice or hospital preferred), and be proficient with EMR/EHR systems; bilingual in Spanish is a plus.
The role emphasizes compassionate care, seamless transitions to Care Navigation, and strong teamwork to meet scheduling and safety expectations.
Qualifications
- RN license in the state of employment is required.
- Minimum of one year nursing experience; hospice or hospital preferred.
- Experience with EMR/EHR systems is required.
- Strong clinical assessment skills, with ED/ICU experience preferred.
- English verbal/written communication; Spanish bilingual is a plus.
Responsibilities
- Complete admissions for all services and inform patients/families empathetically.
- Coordinate with physicians to initiate Plan of Care and care transitions.
- Document per policy and maintain SBAR communications with Care Navigation.
- Work with facilities and care teams to ensure timely, safe discharges.
- Support quality improvement and professional development activities.
