Job summary

Chapters Health System seeks an Admission RN to assess patients at their residence or hospitals and initiate care into our services. You’ll ensure seamless transitions to Care Navigation and act as an ambassador for the organization.

The role requires an active RN license in the appropriate state, at least 1 year of nursing experience (hospice/hospital preferred), and EMR/EHR proficiency. Strong communication, teamwork, and time-management skills are essential.

Qualifications

• Current RN license in the state where the job is based
• Minimum 1 year nursing experience; hospice or hospital experience preferred
• Experience with EMR/EHR systems
• Strong clinical assessment skills; emergency/ICU background preferred
• English communication required; Spanish bilingual preferred

Responsibilities

• Complete clinical admissions for all company services
• Explain services to patients/families and address questions
• Initiate Plan of Care and nursing interventions upon admission
• Obtain physician orders and ensure timely documentation
• Serve as company representative at facilities and coordinate referrals
• Communicate with Care Navigation to ensure timely follow-up
• Provide care during emergencies and when assigned to different locations
