Job summary

Chapters Health is seeking an Admission RN to assess patients at their residence or in hospitals and to initiate care within the organization’s services. The role acts as an ambassador, ensuring seamless transitions to Care Navigation.

The ideal candidate holds a current RN license in Florida, with BLS, at least 1 year of RN experience (hospice/hospital preferred), and EMR/EHR familiarity. Strong communication skills and a valid driver’s license are required.

Qualifications

• Current RN license in the state of employment.
• BLS certification from AHA or Red Cross.
• Minimum 1 year RN experience; hospice or hospital preferred.
• Strong clinical assessment skills; ICU/ED background preferred.
• Experience with EMR/EHR systems.
• Ability to communicate in English; bilingual Spanish preferred.
• Driver’s license and automobile insurance per policy.

Responsibilities

• Complete clinical admissions for all company services.
• Explain services to potential patients and families; address questions empathetically.
• Identify admission objections or barriers and provide responses.
• Educate patient/family on admission requirements and immediate needs.
• Complete informed consents and initial assessment per policy.
• Provide information to physicians and initiate Plan of Care.
