Chapters Health System is seeking an Admission RN in Florida to assess patients in their residency or hospital settings and initiate care into our services. You will serve as an ambassador of the organization and collaborate with Care Navigation to ensure seamless transitions.

The role requires an RN license in the state, at least one year of nursing experience (hospice/hospital preferred), and experience with EMR/EHR systems. A valid driver’s license and bilingual Spanish skills are assets.

Formación

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR system.
• Strong clinical assessment skills with background in ED or ICU preferred.
• Ability to communicate effectively in English; bilingual in Spanish preferred.
• Valid driver’s license and automobile insurance per Company policy.

Responsabilidades

• Completes clinical admissions for all Company services and communicates with patients and families.
• Educates patient/family on admission requirements and immediate needs.
• Initiates Plan of Care and coordinates with Care Navigation.
• Initiates skilled nursing interventions to prevent complications and maximize comfort.
• Obtains Physician orders and documents per Company policy.
• Maintains timely communication with Care Navigation and referring facilities; supports smooth discharges.
• Protects patient confidentiality and adheres to safety procedures.
• Supports quality improvement activities within the organization.
