Job summary

Chapters Health System seeks an Admission RN to assess patients in their residence or hospital and initiate care within our hospice services. This role ensures seamless transition to Care Navigation and acts as the organization’s ambassador. A current RN license in Florida is required, with 1 year of nursing experience and EMR/EHR familiarity.

Travel and driving are involved, with a 10:00 a.m.–9:00 p.m. shift and rotating weekends. Bilingual Spanish is preferred.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice/hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; emergency/ICU background preferred.
• Spanish language skills preferred; English proficiency required.

Responsibilities

• Complete clinical admissions for all services.
• Explain services to patients/families and address questions.
• Educate patients on admission requirements and plans of care.
• Obtain physician orders and document per policy.
• Communicate with Care Navigation and referrals; SBAR communications.
• Assist with discharge planning and follow-ups as needed.
