Job summary

Requires active RN license in the state, at least one year of nursing experience (hospice or hospital preferred), and English communication skills; Spanish is a plus.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• Proficient English communication; Spanish a plus.
• Valid driver’s license and reliable transportation.
• Must work well in a team and manage priorities.
• Ability to perform essential functions including lifting.

Responsibilities

• Represent the Company professionally in care delivered and services provided.
• Coordinate with Care Navigation to ensure timely follow-up.
• Explain admission requirements and address immediate needs.
• Initiate Plan of Care with physicians and address patient needs.
• Deliver skilled nursing interventions to prevent complications and enhance comfort.
• Obtain physician orders and complete documentation per policy.
• Serve as a Company representative at facilities and coordinate referrals.
• Communicate with Care Navigation staff regularly and prepare for next assignment.
