Job summary

Chapters Health System is seeking an Admission Registered Nurse to assess patients at their residence or in hospitals and initiate care into our services. You will ensure seamless transitions to Care Navigation and act as a per­sonal ambassador for the organization.

The role requires active RN licensure in Florida, at least 1 year of nursing experience (hospice or hospital preferred), and proficiency with EMR/EHR systems.

Qualifications

• Current RN license in the state where employed.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems; strong clinical assessment skills.

Responsibilities

• Completes clinical admissions for all services and explains available options to patients and families.
• Initiates Plan of Care with physicians and coordinates care transitions.
• Documents per policy and maintains SBAR communications with Care Navigation.
• Represents the company at facilities and maintains professional relationships with hospitals, physicians and patients.
• Triage and respond to emergencies, ensuring continuity of services as needed.
