Job summary

Chapters Health is seeking an Admission RN to assess patients in their residence or hospitals and initiate care within our Care Navigation system. You will serve as a liaison to ensure a seamless transition and act as an ambassador for the organization. The role requires current FL RN license, BLS, and at least one year of RN experience (hospice or hospital preferred). Proficiency with EMR/EHR and strong communication are essential.

Shift is 10:00 a.m. – 9:00 p.m. with every other weekend.

Qualifications

• Current RN license in the state of work.
• Active BLS certification from AHA or Red Cross.
• Minimum 1 year RN experience; hospice or hospital preferred.
• Strong clinical assessment; ED/ICU background preferred.
• Experience with EMR/EHR systems.
• Ability to communicate in English; bilingual Spanish preferred.
• Valid driver’s license and automobile insurance per policy.
• Reliable transportation and ability to meet schedule.
• Ability to manage multiple priorities and emotional workload.

Responsibilities

• Complete clinical admissions for all company services.
• Explain services to patients/families and address questions empathetically.
• Educate patients on admission requirements and immediate needs.
• Obtain physician orders and initiate Plan of Care.
• Perform skilled nursing interventions to maximize comfort.
• Coordinate referrals and liaise with hospitals, physicians, and facilities.
• Maintain timely documentation and SBAR communications.
• Discharge planning and ensuring safe transitions for referred patients.
