Job summary

Chapters Health System is seeking an Admission RN to assess patients in residents' homes or hospitals and initiate care within our system. The role ensures smooth handoff to Care Navigation and represents the organization professionally in the community.

The position requires an active RN license in the work state, at least 1 year of nursing experience (hospice/hospital preferred), and experience with EMR/EHR systems. A valid driver's license and reliable transportation are required.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience working with an EMR/EHR system.
• Strong clinical assessment skills with preferred background in ED or ICU.
• Ability to communicate in English; bilingual Spanish preferred.
• Mobile driver with valid license and automobile insurance per Company policy.
• Reliable transportation to meet schedule.
• Ability to work in a team and manage multiple priorities under stress.
• Ability to perform essential functions (lifting, bending, etc.) with/without accommodation.

Responsibilities

• Completes clinical admissions for all Company services.
• Explains services to potential patients/families with empathetic, clear communication.
• Identifies admission objections and provides responses.
• Educates patient/family on admission requirements and initiates Plan of Care.
• Initiates skilled nursing interventions to prevent complications and maximize comfort.
• Obtains physician orders and completes documentation per policy.
• Acts as Company representative at facilities and coordinates referrals.
• Communicates with Care Navigation to ensure timely follow-up.
• Facilitates safe discharge and coordinates with aftercare.
