Job Summary

Reporting to the Chief Executive Officer, you will oversee the Social Service Department’s administration and operations, driving effective care management and stakeholder engagement to support residents’ well‑being and independence.

Responsibilities
  • Lead the administration and operational management of the Social Service Department to ensure efficient service delivery and compliance with organizational standards
  • Provide timely and clear updates to next of kin (NOKs) regarding beneficiaries’ status and family matters to maintain transparent communication
  • Prepare and present cases to the Admission & Review Committee to facilitate informed evaluation and approval of admissions to the Home
  • Manage Individual Care Plans (ICP) for residents, clients, and Training in Independent Living (TIL) participants, ensuring personalized care and progress tracking
  • Coordinate admissions, discharges, means testing, and schedule ICP conferences with beneficiaries and their families or caregivers to support seamless transitions and care planning
  • Facilitate regular group work sessions with residents and the multi-disciplinary team to identify and resolve social challenges affecting residents’ quality of life
  • Maintain accurate beneficiary records and manage the Electronic Case Management System (eCMS), reporting ICP updates through the EPES platform to ensure data integrity and compliance
  • Lead publicity and outreach initiatives to relevant agencies and hospitals to increase awareness and participation in the Residents’ Care and TIL programmes
  • Identify and develop job opportunities for beneficiaries to enhance their vocational skills and promote independence
  • Compile and submit accurate data reports to the Ministry of Social and Family Development (MSF) and other relevant agencies to support regulatory and funding requirements
Required competencies and certifications
  • Registered Social Worker with a Bachelor’s or Graduate Diploma in Social Work
  • Minimum of 5 years’ experience in Social Service Agencies, Ministry of Health (MOH), or Nursing Homes (NH) in a similar role
  • Proven experience in leading a small social work team
