CREST (Community Outreach Team) is a vital community safety network supporting individuals at risk of or living with mental health conditions, including dementia, as well as their caregivers.

As a Social Work Associate, you will provide essential support in outreach activities, client care, and programme operations to promote mental wellness in the community. RESPONSIBILITIES
Programme Implementation Support
  • Assist in running targeted outreach and cognitive stimulation activities.
  • Provide administrative, logistical, and site‑related support to the team in running client‑related and CREST activities.
  • Prepare necessary resource materials to be provided to clients and caregivers.
Community Engagement & Outreach
Awareness & Education
  • Plan and support outreach initiatives to corporates and community organisations to promote mental wellness and raise awareness.
  • Assist in organising and facilitating education and training programmes on mental health and coping strategies.
Client Support & Care Coordination
Assessment & Intervention
  • Support in initial assessment of walk‑ins and referral cases.
  • Follow up with intervention plans for clients under guidance.
  • Offer mental health information and emotional support to at‑risk individuals and their caregivers.
Case Management
  • Support care coordination and service linkages for identified clients.
  • Monitor clients' conditions and progress, escalating to senior team members where necessary.
  • Assist in regular follow‑ups and re‑evaluations to ensure continued client well‑being.
Data Management & Reporting
  • Support the team on timely data collation and quarterly report submission.
  • Compile and maintain programme data to track activities and client progress.
  • Assist in preparing reports as required.
QUALIFICATIONS
  • Diploma in social work, counselling, psychology, or other fields related to healthcare and community care.
  • Preferably 2–3 years of healthcare/community care experience.
  • Interest in mental health advocacy and supporting individuals with mental health conditions, including dementia.
  • Good communication and interpersonal skills to engage with clients, caregivers, and team members.
  • Ability to provide compassionate emotional support to clients and caregivers.
  • Organised and detail‑oriented approach to programme support and administrative tasks.
  • Ability to work collaboratively within a team and follow guidance from senior staff.
  • Knowledge of dialects is an advantage.
