Responsibilities

Job description

Responsibilities

• Provide counselling service to patients and caregivers
• Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
• Assist the patient to understand the diagnosis and implications on ADL functioning
• Walk through with patients and caregivers and provide emotional support and motivation for them
• Manage patients' expectations and set realistic and achievable goals
• Arrange for interim care or make referrals for long-term step-down facilities when necessary
• Participate in ward rounds and case conferences to represent patients' social, psychological and economic circumstances.
• Undertake projects and any other duties as assigned by the Management

Requirements

• Master/Bachelor/Post Graduate Diploma in Social Work
• Registered Social Workers with the Singapore Association of Social Work (SASW) - will be an advantage
• Compassion and care for patients
• Good team player with positive attitude, caring and compassionate individual
• Good interpersonal skills, able to communicate with people at all levels, such as patients’ families, government agencies and other professionals
