Job description
Responsibilities
  • Report directly to the Senior MSW.
  • Be involved in the pre-admission work, such as taking on a new referral, liaising with the external agencies and internal team to facilitate an admission to centre based services.
  • Assess & evaluate patients’/families’ psycho-social and financial status.
  • Assist clients to apply for appropriate financial assistance schemes when needed (Medifund, Household Means Testing (Subvention) and other funding schemes).
  • Assist in setting up and upkeep of administrative protocols pertaining to patient needs from onboarding to discharging.
  • Work closely with Home Care team members in the development of the Individual Care Plan.
  • Provide accurate information to clients on policies, procedures and service.
  • Strong case resolution skills to resolve client feedback and complaints on promised actions and that client needs are addressed in a timely and appropriate manner.
  • Work closely with other managers and medical social workers to ensure that all MOH service requirements on financial and quality aspects are strictly adhered to through rigorous and constant internal checks.
  • Assist with logistics, staffing and operational capability of the home care services when required.
  • Communicate clearly and appropriately with clients & business partners, always speaking with the understanding of tone and choice of words.
  • Any other adhoc duties.
Requirements
  • Diploma or Degree in Social Work or other relevant fields desirable.
  • 5-days work week. Rotational weekends (Saturday) required.
  • 1 year in a similar role would be good to have.
  • Preferable experience working with MOH or other relevant stakeholders in the healthcare industry in general.
  • Having experience with IRMS, ILTC Portal and other relevant IT systems would be good.
  • Hands-on experience working with elderly in the community.
  • Able to work independently with little supervision.
  • Proficient in Microsoft Office and comfortable with IT.
  • Able to communicate effectively with elderly.
