Job Description

We are seeking dedicated and compassionate Medical Social Workers to join our team in providing care for our patients and population. As a Medical Social Worker working within the community, you will play a vital role in addressing the biopsychosocial-spiritual needs of patients and their families, facilitating access to resources, empowering individuals and communities, and promoting health and well-being for our population.

Key Responsibilities

• Provide holistic bio-psychosocial-spiritual assessment and interventions for effective end-to-end case management
• Provide counselling and psychoeducation for patients and families coping with illness, disability, and/or end-of-life issues.
• Develop and implement individualized care plans in partnership with health/social care teams to support care transitions and anchor care for patients and residents in the community
• Contribute actively in interdisciplinary team meetings to discuss patient care and progress.
• Assess risk factors and develop strategies to mitigate and address risks, ensuring safety and well-being of clients
• Develop, implement and facilitate group work programmes in the community to foster support and connection
• Collaborate with health and social care providers, grassroots and community partners to join up care, promote health and support ageing in place
• Participate in organisation/department projects and initiatives

Job Requirements

Education/Qualification:
