Job Summary
Purpose of job
- The Manager / Head, Active Ageing provides strategic and operational leadership of the Active Ageing Centre (AAC) to support seniors ageing well in place with dignity, in alignment with MOH and AIC Age Well SG objectives.
- The role ensures effective delivery, governance, integration and continuous improvement of AAC programmes, including health promotion, social participation, functional ability maintenance, early risk detection and community mental health integration, while meeting AIC funding, reporting and quality assurance requirements.
Key Responsibilities Areas
- Provide strategic and operational leadership of the Active Ageing Centre to support seniors to age well in place with dignity, in alignment with MOH / AIC Age Well SG objectives.
- Ensure compliance with MOH / AIC service agreements, including governance, funding conditions, reporting, data submission, and audit requirements.
- Lead the design, implementation, review and evaluation of active ageing programmes covering health promotion, functional ability, social participation and mental wellbeing.
- Drive service integration with Integrated Community Care providers, Community Mental Health teams, healthcare institutions and social service partners to enable seamless care pathways.
- Oversee early identification, care coordination and referrals for seniors with emerging or complex social, mental health or caregiving needs.
- Lead, supervise, and develop a multidisciplinary team, providing practice supervision, performance management and capability building.
- Develop and sustain a strong volunteer pool and community engagement ecosystem, working closely with internal teams and community partners.
- Monitor service quality and outcomes, using data and feedback to drive continuous improvement, innovation and accountability.
- Undertake any additional duties as assigned by the Executive Director.
Requirements
Experience
- Proven experience in Active Ageing Centres, community eldercare or integrated care services.
- Strong track record in programme development, implementation and evaluation.
- Experience working with MOH / AIC funded services and sector partners.
- Demonstrated people management and team leadership experience.
- Experience in inteRAI, case management, groupwork and community engagement is an advantage.
Qualifications
Graduate Diploma / Bachelor’s / Master’s Degree in:
- Social Work
- Nursing
- Management and Relevant Allied Healthdisciplines
