Qualifications

• Degree in Social Work or a Graduate Diploma in Social Work.

• Experience in means testing and healthcare setting is advantageous.
• Conversant in local dialects and/or Malay, Mandarin, Tamil is advantageous.

Responsibilities

• Conduct biopsychosocial and financial assessments for clients and caregivers.
• Develop care plans and coordinate follow-up actions to address needs.
• Work with multidisciplinary healthcare partners to ensure smooth care transitions.
• Provide direct counselling and emotional support to clients and caregivers.
• Conduct home visits independently or with the Home Care team.
