Job Purpose
Responsible for translating narrative descriptions of diseases, injuries and procedures into codes, based on a pre-defined Casemix classification system.
Duties and Responsibilities
- Primary Responsibilities and Duties (80%)
- To accurately abstract relevant data and translate the principal diagnosis, secondary diagnoses and surgery/procedures of all inpatient and day surgery discharges into ICD 9 CM codes from discharge summary/case notes/operation reports/histology reports in accordance with the national and hospital coding standards within the stipulated time frame for subvention.
- To interact closely with clinicians for clarifications/confirmation/feedback on documentation of principal diagnosis, secondary diagnosis and surgery/procedure/treatment if necessary that may affect coding accuracy and correct subvention.
- To keep abreast of latest coding standards and update coding knowledge/coding skills.
- To work independently and act as a mentor to junior coders.
- Secondary Responsibilities and Duties (20%)
- To manage and track daily uncoded/coders workload report.
- To work closely with the supervisor so as to continuously improve the current work processes in Casemix office.
- Participate in quality assurance programs including internal audit programmes.
- Senior Coders to be involved in training of new coders/internal audits.
- Able to understand departmental operational workflow and resolve any issues independently.
- Carry out any other projects and duties as assigned.
Job Specification
- Qualification: Basic Medical Degree or State Registered Nurse qualification.
- Experience:
- At least 2 years experience in Clinical Coding.
- Skills:
- Ability to work independently and a good team player.
- Ability to coach and mentor.
- Good interpersonal skills, attitude and aptitude.
- Computer literacy.
