Key Highlights
  • Office Hours: Monday-Friday
  • Nearest MRT: Caldecott Station
  • Be a part in coordinating and managing cases, ensuring proper closure and seamless transfer of cases to community partners.
Key Responsibilities
  • Case Management and Transfer:

Manage cases from initiation through to closure, working closely with internal teams and external partners.

Coordinate meetings and case triages with relevant agencies to ensure smooth transitions and follow-ups.

Administrative Support

Provide administrative assistance in case closure and transfer processes, ensuring all documentation is accurate and up-to-date.

Draft and submit referral forms, closure reports, and relevant communication to agencies.

Update case management systems with necessary data and documents.

Collaboration & Communication

Liaise with internal teams, external agencies, and other stakeholders to ensure effective coordination and case progression.

Ensure all involved parties are kept informed and updated.

Additional Duties

Perform any other related administrative tasks as assigned by the supervisor to support case management operations.

Key Requirements
  • Degree in Social Work or related fields.
  • Strong ability to work independently, as well as collaboratively within a team.
  • Attention to detail and experience in handling data and administrative duties (proficient in Microsoft Word and Excel).
  • Excellent communication skills with the ability to interact effectively with both internal and external stakeholders.
  • Experience in case management or related operations will be an advantage.
  • Positive working attitude, with a willingness to learn and grow within the role.
