As a Care Manager (Chronic Disease Management), you will be responsible for the following duties:

• This role encompasses a broad and multifaceted set of responsibilities within clinical care, quality improvement and management.
• Identify patients requiring care management through data review, referrals, and assessments.
• Conduct comprehensive evaluations to determine patient needs and health status.
• Monitor patients regularly using key indicators such as blood glucose levels and HbA1c.
• Implement and adjust care plans in response to patients’ progress and changing conditions.
• Develop and track outcome indicators to ensure effective management of both clinical and non-clinical results.
• Contribute to the development and continuous improvement of care programs and protocols.
• Maintain patient confidentiality and ensure accurate, complete documentation.
• Assist in creating and updating disease management guidelines, standards, and procedures.
• Conduct audits of care management documentation to identify gaps and opportunities for improvement.
• Participate in research activities and integrate evidence-based findings into practice.
• Perform data analysis to support clinical pathway development and report on program outcomes.
• Supervise and mentor junior care managers and nursing staff.
• Conduct performance appraisals and provide constructive feedback to support professional development.
• Oversee support group activities and facilitate patient recruitment for chronic disease programs.
• Address workflow issues related to chronic patient management.
• Monitor productivity and performance, providing coaching and guidance as needed.
• Any other ad hoc duties as assigned.

Requirements

• Possess a Diploma / Degree in Nursing with a relevant Advanced Diploma in Nursing Specialty and a valid full registration with Singapore Nursing Board.
