Responsibilities

  • Lead and evaluate patient care activities according to policies.
  • Coordinate with healthcare team members on patient care.
  • Ensure timely access to treatments.
  • Update patients’ medical records.
  • Communicate patient management plans with doctors.
  • Provide general counselling on treatment side effects.
  • Delegate instructions to team members.
  • Identify service gaps and provide solutions.
  • Encourage team participation in service delivery.
  • Contribute in multidisciplinary work groups.
Job description
Job Responsibilites
  • Lead and evaluate patient care activities to ensure care delivery is in accordance to established policies and procedures and guidelines
  • Coordinate and collaborate with patients, other healthcare team members, patient’s family and community-based agencies / institutions on patient care and discharge needs
  • Coordinating withteam members to ensure timely access to treatments
  • Ensuring patients’ medical records are complete/updated for the day
  • Communication with doctors and team members on patient management plan
  • General counselling on treatment side effects, lifestyle modification, self-care advice, addressing patient concerns, and psychosocial support
  • Coordinate and delegation of instructions to team members on clinical and patient matters
  • Collaborate with Nurse Manager / Operations Manager in identifying service gaps and provide expert opinion and providing solutions
  • Encourage a participatory team approach to service delivery, canvas ideas and strategies to enhance the service delivery of the team
  • Contribute in multidisciplinary work groups andprocesses to reflect nursing leadership role
