Helper Heroes - Virtual Assistant is seeking a detail-oriented Home Health Intake Coordinator to manage referrals from receipt through readiness for admission. This remote role supports U.S.

home care agencies and requires strong follow-up, organization, and proactive communication with patients and providers. The ideal candidate has 1–2 years in home health intake or related areas, can enter data accurately into EMR systems, verifies insurance, and coordinates with clinical teams.

Qualifications

• 1-2 years in home health intake, referrals, admissions, or similar healthcare settings.
• Familiarity with home health orders, insurance verification, and medical documentation.
• Knowledge of Medicare home health and F2F requirements is a plus.
• Experience using EMR or referral-management systems.
• Strong communication, organization, attention to detail, and follow-up skills.

Responsibilities

• Manage the full intake workflow from referral through admission and scheduling.
• Process and enter referrals accurately into EMR (Axxess).
• Review referrals for completeness: demographics, insurance, diagnoses, orders.
• Identify and resolve missing or conflicting information before admission.
• Verify insurance coverage and obtain required authorizations.
• Confirm the appropriate following physician and process orders.
• Fax, track, and follow up on orders and documentation with offices.
• Contact patients/caregivers to verify information and confirm acceptance.
• Coordinate with hospitals, physician offices, case managers, and teams.
• Schedule clinicians based on discipline, availability, and payer needs.
• Upload and maintain documentation and records in EMR.
• Track open referrals and follow up on barriers until resolution.
• Prioritize time-sensitive referrals and escalate delays.
• Provide clear referral status updates to teams.
• Maintain documentation of actions, barriers, and next steps.
• Apply Medicare home health guidelines, OASIS, and compliance expectations.
