Job summary

ReadyTeam in the Philippines seeks an Intake Coordinator for our Virtual Home Health / Hospice team. You will support referrals and admissions with accuracy, organization, and efficiency from start to patient start.

Responsibilities include reviewing referrals, verifying documents, coordinating with sources, entering patient details, and updating clinical and administrative staff to improve turnaround times and patient experience. This is a training-friendly, month-long onboarding.

Qualifications

Our Virtual Home Health / Hospice Intake Coordinators help home health / Hospice agencies manage the referral and admission process with accuracy, organization, and efficiency. They support the intake workflow from the initial referral through admission coordination, helping agencies respond quickly, reduce delays, and maintain clear communication with patients, providers, and internal teams.

Responsibilities include receiving and reviewing referrals, verifying required intake documents, requesting missing clinical records, coordinating with referral sources, entering patient demographics, organizing insurance and authorization information, assisting with scheduling start-of-care visits, and communicating updates to clinical and administrative staff.

With strong attention to detail and familiarity with home health workflows, our Virtual Home Health / Hospice Intake Coordinators help agencies improve referral turnaround time, maintain organized documentation, and create a smoother admission experience for patients and providers.
