Pathways in Henderson, Auckland, is seeking a full-time team member to support tangata whai ora with short-term, intensive care. You will join a Waitemata Acute Community Mobile team, reporting to a team coach, and collaborate with the Te Whatu Ora Waitemata Acute Clinical team.
The role runs Tuesday–Saturday, 11:30am–8:00pm, based in Henderson. You will advocate, connect clients to resources, support community integration, and assist with appointments and medication support.
Work alongside tāngata whaiora to maintain physical, emotional & spiritual wellness.

You will report to our team coach and work in our Waitemata Acute Community Mobile team. We are a team of 9 support workers across three sites. We are a unique responsive service that co-locates and collaborates closely in partnership with the Te Whatu Ora Waitemata Acute Clinical team to provide support for tangata whai ora to offer short-term, intensive and intentional support for people who are experiencing distress of unwellness.
This is a full-time position working Tuesday - Saturday, 11:30am - 8:00pm, based in Henderson, Auckland.
