If you're interested in joining Routes as a Case Manager in Merseyside/Warrington, here’s what we’re looking for:

• A passion for putting people first.
• Strong organizational and time management skills.
• Understanding of CQC requirements.
• Experience managing complex care packages involving clinical interventions (e.g., spinal injuries, brain injuries, MND, MS).
• Experience leading a team of healthcare workers.
• Must be a driver with own vehicle.

Your core responsibilities:

• Managing a caseload of complex clients with clinical needs.
• Coordinating and leading care package transitions and startup processes.
• Upholding high standards of care with a person-centered approach.
• Prioritizing individual wellbeing and safety.
• Handling complaints and safeguarding issues proactively.
• Developing care plans and risk assessments.
• Providing support through observations, mentoring, and reviews.
