We are looking to recruit enthusiastic Band 5 Registered nurses who will deliver a high-quality patient care and thrive on the opportunities this post offers. Community health care is changing and fast developing, and it is an exciting time to be a part of the Discharge Business Unit! The posts advertised will be for allocation on one of the 4 community wards, 2 at Sir Robert Peel, Tamworth and 2 at Samuel Johnson Community Hospital, Lichfield.

• Provide safe, effective high quality standard of care to elderly patients within the 24 bedded unit.
• Utilise Resources in in an effective and efficient way.
• Work within the multidisciplinary team to help support the patients plan of care during their hospital stay.
• Ensure appropriate referrals are made to safeguarding team, tissue viability, and any other profession to support inpatient care management.
• Participate in practice development and support changes within improvement projects.
• Maintain the health and safety of patients.
