About the role

We are seeking a dedicated and motivated Social Work Officer to join our Mental Health Team. This is an exciting opportunity to make a real difference in the lives of adults living with mental health conditions. The successful candidate will play a key role in delivering high-quality social care interventions in line with statutory duties and local authority functions.

Key Responsibilities

  • Carry out Care Act Assessments and Reviews for adults with mental health needs.
  • Manage a caseload effectively, ensuring timely and appropriate interventions.
  • Work as part of a supportive team, attending team meetings and contributing to collaborative decision-making.
  • Represent social care within a multi-professional setting, working closely with clinical teams and attending relevant professional meetings.
  • Undertake welfare checks, home visits, and other duties aligned with local authority responsibilities.
  • Work flexibly to meet the needs of adults requiring social care support.
  • Complete written assessments to a high standard, demonstrating strong written and verbal communication skills.
  • Ensure compliance with statutory timeframes and guidelines.
  • Manage competing demands effectively and maintain accurate records.
