Job Advert - Social Worker (Adult Social Care Team)

Location : Wokingham

Rate : £35.89 per hour

Team : Adult Social Care (18+)

Key Responsibilities

• Complete Care Act assessments, reviews, and support plans
• Undertake safeguarding enquiries (S42)
• Work with individuals with complex needs and their carers
• Engage in multidisciplinary working across health and community services
• Support people to achieve personal outcomes and maintain independence
• Carry out office-based work at least once per week and visits on other days

Candidate Requirements

• Qualified Social Worker with a minimum of 3 years' experience in Adult Social Care
• Experience in dementia, drug / alcohol issues, homelessness, hoarding, safeguarding, carers assessments, community work, and MDT working
• SWE registration
• Car driver with access to a vehicle
• Ability to work both independently and collaboratively
