Overview

Are you looking for a role that presents a real challenge every day? We are seeking Urgent Care Practitioners to join the Mental Health Liaison team. This role sits at the interface between physical and psychological health, bringing knowledge and expertise to general hospital colleagues. You will conduct specialist and timely mental health assessments and provide treatment for patients attending the General Hospital. Our service supports patients with a wide range of problems, including self-harm, adjustment to illness, and physical and psychological co-morbidities. The team handles a variety of presentations, including those new to mental health services.

Role details

This post offers the Trust\'s £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

Responsibilities
  • Provide expert mental health assessments and treatment, working with general hospital colleagues both informally and through teaching sessions.
  • Contribute to the management of common mental health problems and support integration with other health and social care services.
  • Support the Older Persons Liaison Team and provide assessments and support to individuals aged 18 and over.
  • Foster excellent communication and relationships with patients, carers, general hospital staff and other parts of the health and social care system.
  • Work autonomously while contributing to the team; adapt to changing circumstances and work under pressure.
  • Support the 24/7 Coastal Urgent Care Pathway as it moves toward CORE 24 standards; flexible hours may be required.
  • Work across Urgent Care Teams (Crisis Resolution Home Treatment Team and The Haven) as needed.
  • Ensure compassionate, person-centered assessment and timely care plans across acute settings, The Haven, General Hospital and patients home.
Requirements
  • Registered professional (Nurse RMN/LD, Social Worker, or Occupational Therapist).
Knowledge / Experience (Essential)
  • Knowledge of the Mental Health Act (1983/2007).
  • Knowledge of the Mental Capacity Act (2005), including practical knowledge of FACE documentation, Deprivation of Liberty Safeguards (DOLS) and Best Interest decisions.
  • Child and Adult safeguarding procedures.
  • Experience in organising and prioritising workload.
  • Ability to enhance care through evidence-based knowledge.
  • Commitment to professional development relevant to the care group.
