A large healthcare provider in Telford is seeking an Urgent Care Practitioner to join their clinical team. The role involves assessing and triaging patients, providing definitive treatment, and managing both acute and chronic conditions. Assess and triage patients, including same daytriage and as appropriate, provide definitive treatment (including prescribingmedications following policy, patient group directives, NICE or make necessary referralsto other members of the primary care team.
We are a large supportive training practice based inTelford with 24,800 patients and based over two sites. We have a large clinicalteam consisting of 16 GPs and30 clinical staff, including nurses and alliedhealth professional all supported by the administrative team.
Assess patients presenting with acute illness or forreview of acute exacerbations of chronic disease conditions.
Undertake home visits with an elderly care homesetting for acute illness.
Diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis.
Clinically examine and assess patient needs from aphysiological and psychological perspective, and plan clinical careaccordingly.
Prioritising health problems and interveningappropriately, to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care.
Proactively identify, diagnose and manage treatmentplans for patients at risk of developing a long-term condition asappropriate.
Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug based treatment methods into amanagement plan.
