Main duties of the job

As a Team Leader at Avery Healthcare, you will support the home management team in delivering high-quality, person-centred care. You will be responsible for implementing, monitoring, and evaluating care plans, assisting in smooth resident discharges, and ensuring the well-being of each resident. You will also lead and monitor staff, ensure safe systems of work, and promote a caring, supportive, and inclusive environment.

Job responsibilities

Other responsibilities will include:

  • Leading in the person-centred care planning process by implementing, monitoring, undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping residents’ next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records amended to reflect any variation.
  • Practising safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
