A leading support organization is looking for a Support Coordinator for hospital discharge services. The role involves visiting hospitals to assist customers experiencing homelessness, creating discharge plans, and collaborating with health and housing services. Working days, Monday to Friday 9am to 5pm. However, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
Able to use technology for creating and updating support plans, complete online learning and collaborate with colleagues.
A vehicle insured for business use and a valid driving license (we'll pay for business mileage).
