A health charity in the UK is seeking passionate Support Coordinators to assist stroke survivors in their recovery journey. The role involves helping individuals identify their goals, providing personalized support, and working closely with healthcare professionals. Strong interpersonal and organizational skills are essential, along with a caring background. This position requires travel across Norfolk and Waveney, and candidates must have the right to work in the UK.
This is a fixed-term contract until 30 September 2024. Our services are project-funded, we currently have funding for this service until 30 September 2024.
We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to be part of the team delivering our Stroke Recovery Service based in Norfolk and Waveney. As a result of an increase in local funding to support stroke services across Norfolk and Waveney, we are currently looking to recruit five new support coordinators to help us deliver our strategy and make a real difference.
Reporting to the Service Delivery Coach in the East of England, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centered approach, enabling them to maximise their recovery and improve communication.
This role requires frequent travel across a large geographical locality to visit people at home and in community settings across the Norfolk and Waverley area. Candidates must be able to demonstrate how they can meet this requirement of the role.
