A local healthcare organization in Weymouth seeks a Smoke Stop Practitioner to enhance smoking cessation services. The role involves developing and delivering support sessions, engaging with community members, and promoting health and well-being. Ideal candidates should have experience managing caseloads, possess excellent communication skills, and be NCSCT accredited. This position offers flexible hours with a pay range of £14.06-£15.43 per hour.
In Weymouth and Portland PCN at least 13% of the entire population smoke, with even higher proportions in the most deprived groups.
We are intending to increase our smoke stop offer - and are looking for a smoke stop trained individual who is able to work across the PCN to deliver additional clinics. We will only be offering nicotine replacement therapy in these clinics, no oral medications - and we will follow the 'not one puff' strategy.
The clinics will be a combination of face to face and telephone appointments as appropriate. The intention is that they will be in a variety of settings , not just in GP Surgeries. There may be evening (telephone only)and Saturday morning clinics as well.
This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. It focuses on engaging with people in communities and offering them practical support to change their behaviour to achieve their own choices and goals. The post holder will provide smoking cessation advice and support to individuals and groups of people who want to stop smoking.

Two Harbours Healthcare is a not for profit organisation led by local General Practices in Weymouth and Portland. Our goal is to work more closely together (and with stakeholders) to share expertise, resources and provide services for the NHS.
Toplan, develop and deliver smoking cessation advice and support sessions,including advice on stop smoking medications, working within recommendedprotocols and guidelines. This willinvolve individual, telephonic, on-line and group work sessions in a variety ofsettings and venues, working closely with the central Yorkshire Smokefreetelephonic Hub staff as required.
Developindividually tailored support plans including recommending treatments based onassessed needs.
Provideinformation to people about stopping smoking and health-related issues in linewith best guidance and protocols.
Providegeneral health and lifestyle advice and support to people using the servicee.g. healthy eating, physical activity, stress management. This will include individual and group basedinterventions, signposting and referring to other health, community andvoluntary services.
Responsiblefor the delivery of stop smoking support to specific client groups (e.gpregnant women, routine and manual workers)
Deliververy brief, brief and awareness training in stopping smoking as required.
Tobe involved in practice awareness raising sessions in Primary Care andCommunity settings.
Tobe involved in monitoring and evaluation of attendance at smokefree supportsessions and contribute to the completion of the minimum data set requirement.This will include following up quitters who do not attend for planned support.
Tobe involved in the marketing and promotion of the service alongside othercolleagues. This may involve attending events on evenings/weekends.
Undertakespecific project and/or development activities.
Maintainaccurate client electronic records.
Respondeffectively to referrals within stated timescales self-referrals from theindividuals, and those from colleagues /other workers/GPs.
Ensureadministrative duties are carried out efficiently and effectively in order tomeet the needs of the service.
Tobe involved in monitoing and evaluation of client feedback, offeringrecommendations to improve services.
Workingas part of a Team
Alertspecialist advisors or line manager to any issues in work (including concernsabout individuals or work in the community) and maintain confidentiality
Activelycontribute to ways in which local services can be improved, supportingimprovements in the health and well-being of the local community and barriersthat individuals are experiencing in changing their behaviour.
Contributeto the development of the Smokefree service targets and any plans that may bedeveloped by attending meetings as appropriate with co-workers and sharinginformation.
Tomaintain good working relationships with all levels of staff.
Towork closely with other team members and health promotion colleagues across Weymouthand Portland and the wider community.
Toliaise with others in the health and social care community to promote the stopsmoking service and encourage appropriate referrals.
