Job description
Overview

An exciting opportunity has arisen for a full time Band 6 position working with our Early Supported Discharge for Stroke service in Essex ( 2.5 days ) and Havering Older Adult Mental Health Team 2.5 days) . This post has the benefit of working closely with our well-established and experienced Stroke Hub Team whilst sitting within our well supported and friendly SLT team of Band 7, 6's and SLTA's who provide a highly specialist level of service to our Community Inpatient Wards, Early Supported Discharge (ESD) for Stroke and General Community caseloads.

Key Responsibilities
  • Play a key role withing the Intensive Havering Older Adult Team, providing specialised speech and language therapy services to individuals with Dementia.
  • Deliver community-based support, aiming to prevent unnecessary hospital admissions, offer education to care homes and facilitate the individual's well-being and recovery within their familiar environment.
  • Conduct assessment and comprehensive therapy programmes for both communication and dysphagia impairments, delivering rapid assessment and intensive rehabilitation with goal focused therapy for up to 6 weeks.
  • Work with both teams, demonstrating excellent organisational skills, strong communication, and proactive involvement as an MDT member.
Salary & Benefits

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,303 to a maximum of £2,198 p.a. pro rata for part time).
