Job description

An exciting opportunity has arisen for a Specialist Physiotherapist who will be based on the Community Assessment Unit based at RAEI, Wigan. The unit has 21 beds and 6 assessment chairs providing holistic specialist Geriatric Assessments. Our focus and priority is on the timely, safe and effective discharge of frail patients back into their normal place of residence. NHS experience is essential.

The successful candidate will be responsible for:

  • Providing a high standard of intervention to support individuals who are medically well enough to be discharged from hospital.
  • Providing mobility assessments and completing baseline assessments for patients within the unit.
  • Providing excellent communication with patients, families and colleagues.
  • Allowing safe provision of equipment to patients being discharged.
  • Acting as a source of knowledge for patients presenting with a range of long term conditions and needs.
  • Taking a role in the specialist assessment and treatment of patients who may have complex and chronic presentation, and determining clinical diagnosis and treatment indicated.
  • Managing a defined caseload using evidence-based/client-centred principles to access, plan, implement and evaluate interventions.
  • Providing leadership and clinical supervision for other junior staff as delegated by manager and/or professional supervisor.
  • Working collaboratively within the services across Wigan and liaising with professionals and voluntary agencies to optimise patient care.
