AHP Frailty at the Front Door

We are delighted to share this exciting new opportunity for Physiotherapy staff to join us to continue to develop our AHP front door frailty service within Raigmore Hospital, Inverness. This new service works across the interface between acute and community settings supporting those patients identified with frailty to maximise their independence and wellbeing within their local communities, to deliver early intervention to support timely discharge and to prevent avoidable hospital admission.

This post is part time and will form part of the MDT team delivering 12 hour day shifts over 7 days. This post will focus predominantly on frailty assessment beds and the initiation of MDT comprehensive geriatric assessment but will work flexibility, dependent on service need.

We are seeking an enthusiastic and innovative Physiotherapist with excellent clinical skills, in particular within the field of frailty, excellent organisational and communication skills and experience of working in a multidisciplinary team. An interest in service development within a fast paced and changing health and social care system is also be desirable.

The successful candidates will work in partnership with the MDT throughout various Raigmore acute settings, as well as the community integrated teams, to ensure patients receive the right care, in the right place, at the right time.

Joining our service offers candidates the opportunity to work as part of a supportive team. Formal and informal supervision is available and individual development through performance review and CPD activity is actively encouraged.
