Job summary

A local authority in Lancashire is seeking an experienced Adult Social Worker to join their team. This role involves triaging hospital discharges, applying Care Act eligibility criteria, and conducting Mental Capacity Assessments. Qualifications

• Experience in adult social care or hospital discharge pathways required.
• Strong knowledge of Care Act eligibility criteria needed.
• Ability to conduct Mental Capacity Assessments confidently.

Responsibilities

• Work within SPA to triage hospital discharges.
• Apply Care Act eligibility criteria.
• Complete Continuing Health Care Assessments.
• Conduct Mental Capacity assessments as per MCA 2005.
• Support transitions through hospital discharge pathways.

• 🏥 Work within the SPA - triage for hospital discharges

• 🧠 Conduct Mental Capacity assessments in line with MCA 2005
• 🏡 Use your knowledge of hospital discharge pathways to support smooth transitions

What We're Looking For:

• Experience in adult social care or hospital discharge pathways
• Strong knowledge of Care Act eligibility criteria and CHC processes
• Ability to complete Mental Capacity Assessments confidently
• Excellent communication and decision-making skills
