Qualifications

  • SWE registered with proven experience in social care or healthcare settings.
  • Experience with person-centred and strengths-led approaches.
  • Understanding of the Care Act, Children Act, Mental Capacity Act and transition guidance.
  • Ability to recognise and respond to safeguarding concerns.
  • Confident engaging with young people, families and partner agencies.
  • Background in working with young people with SEND or complex needs.
  • Previous involvement in Preparing for Adulthood or transitional social care work.
  • Experience in assessing complex needs and developing effective support plans.

Social Worker / Preparing for Adulthood

An exciting opportunity for a Social Worker to support young people transitioning into adult life, using person-centred and strengths-based social care practice.

SALARY: £42,839 to £49,282 per annum + Benefits

LOCATION: Hybrid split between working from home and the office in Wokingham with some visits/meetings across the area if needed

JOB TYPE: Full-Time, Permanent

WORKING HOURS: 37 hours per week

As a Social Worker you will take a person-centred, strengths-based approach, assessing needs, connecting people with support and ensuring safeguarding responsibilities are met.

DUTIES

Your duties as the Social Worker include:

  • Care Act Assessments: Assess and review needs, providing clear guidance and tailored support
  • Strengths-Based Practice: Build positive, person-centred relationships that promote independence

  • Problem Solving: Analyse complex information and contribute creative, practical solutions
CANDIDATE REQUIREMENTS
  • Registered Social Worker: SWE registered with proven experience in social care or healthcare settings
  • Strengths-Based Practice: Experience with person-centred and strengths-led approaches
  • Legislation Knowledge: Understanding of the Care Act, Children Act, Mental Capacity Act and transition guidance
