Job summary

A leading healthcare organization in Doncaster is seeking a dedicated professional to work with rough sleepers and hard-to-reach communities in the substance misuse field. Job description

The outreach team will engage with the rough sleepers and the hard‑to‑reach community, as well as those with physical health needs and support them to enter the treatment system and the wider local community services. The successful candidate will devise a holistic care package, following an assessment of risk and need. You will be required to work flexibly to meet the needs of this group.

If you have relevant experience in the substance misuse field, a relevant degree in psychology, sociology, social work or a diploma in Adult Care or equivalent we would like to hear from you.

If you are experienced by working in the field but do not have the formal qualifications, we provide a comprehensive package of accredited qualifications and for newly qualified Social Workers we support the ASYE programme.

Main duties of the job
  • Undertake comprehensive assessments in order to assess the eligibility and suitability, strengths and needs of a service user in relation to the service.
  • To ensure recovery care plans are devised, monitored, reviewed and updated within the service to ensure that service users’ needs are met in a timely and appropriate manner.
  • Ensure care packages including, where appropriate, psychosocial and psychological interventions specifically aimed at engagement are delivered.
  • Conduct risk/need assessments and implement risk management/care plans and review in line with policy and procedures.
  • Liaise with other voluntary, community and health and social care professionals where necessary and appropriate.
  • Function as part of a multi‑disciplinary team, contributing to team meetings as required.
  • Case‑manage in line with a harm reduction, adult safeguarding & recovery focused model.

Aspire is a partnership between ADS and Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH).
