Job description
Overview

Senior Practitioner sought to join the Home Treatment Team in Sheffield. The Sheffield Home Treatment Team provides crisis support in the community, operating 7 days a week (including Bank Holidays) and is geographically split into 4 sectors aligned with PCMHT and CMHT. The post holder will work closely with three other senior practitioners to provide leadership, clinical expertise and supervisory support, with cross-cover as required. The Home Treatment Team offers acute mental health intervention to adults (primarily 18+) in their own homes as an alternative to hospital admission, functioning as both step-down and step-up care with gatekeeping and least restrictive practice. The MDT includes Psychiatrists, ANPs, Nurses, Support Workers and other professionals.

Responsibilities and duties

Principal Duties

  • Provide leadership with the Service Manager and Leadership for managing and developing staff from a range of professions through regular supervision and appraisals.
  • Be a positive role model to promote a conducive working environment.
  • Work with the Leadership Team to develop effective team working and evidence-based practice within the service.
  • Ensure the service adheres to Care Pathways and service-wide systems and processes for health and social care delivery.
  • Contribute to development of service policies and protocols in conjunction with other senior practitioners and managers.
  • Ensure effective governance structures to support delivery of care; deputise for the Service Manager in their absence if required.

Other Duties

  • Ensure continuity of service and maintenance of standards when deputising for the Service Manager.
  • Maintain own professional competence and ensure compliance with SHSC policies (Health and Safety, Equal Opportunities, Confidentiality, GDPR).
  • Adhere to professional codes of conduct.

Professional and Advisory Role

  • Advise on the role and function of the service and provide clinical and professional advice to a broad range of professionals.
  • Maintain relationships with professional leads and develop partnerships with primary care and community mental health services.
  • Develop and maintain clinical standards in line with local and national policy.

Service Development

  • Audit practice within the team and ensure staff undertake appropriate training.
  • Maintain contemporary knowledge of educational developments and engage in service improvement initiatives.

Person Specification

Registered Clinician — Essential

  • Registered Mental Health Nurse

Experience — Essential

  • Awareness of current community mental health issues, policies and developments; understanding of local, directorate and national targets.
  • Experience of supervising staff and ability to make skilled decisions in distressing, high-emotion situations.
  • Experience of mental health work in a community/crisis setting.

Desirable

  • Experience with culturally diverse populations; experience in Crisis Services; experience with service improvement within Clinical Governance; ability to negotiate with key stakeholders.

Knowledge / Skills — Essential

  • Knowledge of risk assessment and risk management; knowledge of local and national mental health policy; comprehensive knowledge of the Mental Health Act and related legislation; strong oral and written communication; ability to provide clinical and managerial leadership and role modelling.

Desirable

  • Knowledge of audit and evaluation processes.

Training / Qualifications — Essential

  • Active registration as a Mental Health Nurse and continued professional development in leadership and management.

Desirable

  • Management/leadership qualification or experience; Non-Medical Prescriber.
