A leading healthcare organization in Liverpool is seeking a Senior Practice Nurse who is an experienced autonomous practitioner to deliver advanced clinical care for complex chronic conditions. The ideal candidate will manage clinical care independently and contribute to policy development and staff training. They should possess strong expertise in General Practice and be a registered nurse with prior experience in a GP setting. This role offers a range of incentives including mentorship and support for staff wellbeing.
The Senior Practice Nurse is an experienced autonomous practitioner and non medical prescriber with strong expertise in General Practice and a thorough understanding of the Quality Outcomes Framework and wider quality targets. They deliver advanced clinical care for complex chronic conditions, particularly for housebound patients, while working effectively within multi professional and multiagency settings. Operating at an advanced level of practice, they provide leadership through staff training, supervision, and support, and contribute to clinical audits, research, and service improvement. The role also involves developing and updating local protocols, guidelines, and procedures to ensure high quality, evidence based care.
Undertake complex general practice nursing interventions across a wide range of clinical presentations, applying advanced practical skills and a critical understanding of the theories underpinning primary care.
Assess patients capacity for informed consent and support colleagues in developing this competency.
Apply highly developed specialist knowledge of longterm conditions, health behaviours, and evidence based interventions, including the use of technology to support patients at home.
Independently manage clinical care episodes from initial presentation through to completion, including referral, admission, discharge, or ongoing homebased care.
Use advanced therapeutic knowledge to prescribe and evaluate pharmacological and nonpharmacological treatments, assessing patient concordance and adjusting care plans accordingly.
Challenge practice, systems, and policies constructively and objectively to support continuous improvement.
Contribute to the development and review of local policies, pathways, and procedures.
Design, deliver, and evaluate training and education for individuals and groups, working collaboratively across disciplines to support interprofessional learning.
Demonstrate originality and innovation in service development, ensuring safe implementation of new guidelines, policies, and models of care.

Demonstrate problem-solvingskills underpinned from perspectives, for example in research processes,service and quality improvement techniques, educational theory, or leadershipand management theory.
Participatein the planning and implementationof development programmes.
Focus on theimprovement of patient safety by developing systems to disseminate learningfrom incidents and follow up to ensure best practice is embedded in delivery ofcare.
Act as anexperienced work-based learning educator/assessor by providing advice andsupport to other practitioners and build capability and capacity to supportlearning in practice settings.
Participate inmonthly clinical education meetings.
Leadership & Management:
They may delegate work to othermembers of the multidisciplinary team and take accountability for the delegatedactivity.
Able to assimilate informationfrom a range of sources and ensure complex decisions reflect the analysis ofthese different perspectives even when limited information is available.
Is able to participate instrategy development, presenting a positive role model for general practicenursing, and ensuring this translates into practice development to improve thequality of care.
Able to apply the theoretical perspectives of change management tocreate an environment for successful change and practice development andutilise conflict management and resolution strategies where appropriate.Demonstrate and evaluative an outcomes-based approach to practice anddevelop strategies to share this with a wider audience.Is able to appreciate the broadest context of clinical governance andinitiate and support others, including service users, to be involved in a rangeof quality assurance and monitoring activities.
Ensure that quality and auditcycles are completed and results and learning are fed back into practice.
Develop processes for monitoring clinical effectiveness and efficiencyto enhance management of resources.
Where required undertake significant event auditing (or equivalent) andbe skilled in undertaking objective investigations and in writing objectivereports following the completion of the investigation and in presenting thisinformation orally if required.
Able to work effectively with a wide range of professionals andagencies and participate in multi- professional/agency strategies.
Evidence, Research &Development:
To lead at least one special area of expertise to be seen as a localexpert and role model able to articulate the most contemporary evidence,approaches to practice and management.
Identify and utilise skills and knowledge of staff to support orundertake research-related activity such as audit, evaluation and widerresearch for the benefit of the organisation.Monitor impact of evidence utilisation.
Safeguarding Children andAdults:
The organisation has a zero tolerance approach to the abuse ofchildren, young people and vulnerable adults. All employees are required topromote and safeguard the welfare of children and young people and comply withthe Local Safeguarding Children Board Procedures and the Children Act (1989,2004). In accordance with the CCG and Local Safeguarding Adult Boardpolicy all staff must ensure the health and wellbeing of vulnerable adults isappropriately safeguarded.
