A healthcare organization in England is seeking a Senior Assessor to provide high-quality, person-centred care within the Home First service. The Home First service provides short‑term, responsive and flexible support aimed at helping individuals regain and maintain independence in their own homes.
The service plays a crucial role in preventing unnecessary hospital admissions and facilitating timely hospital discharges. Working in close collaboration with social care, community health teams and acute hospitals, Home First supports patients during transition periods by offering enablement‑focused care.
The service aligns with national integrated care strategies and is underpinned by a person‑centred approach, empowering individuals to achieve outcomes that promote recovery, autonomy and well‑being in their home environments.
The Senior Assessor plays a pivotal role in delivering high‑quality, person‑centred care within the Home First service.
They provide advanced assessment, complex case management and operational support, while also guiding and developing colleagues to maintain excellence across the team.

Please note: the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
