This is an exciting opportunity to contribute to the evolution of urgent care services in Cornwall.
The GP role will be based in the same day emergency care clinic working alongside experienced frailty nurses and advanced clinical practitioners to assess patients with an urgent care need and to assess suitability to be cared for at home with the support of the virtual ward. The clinical space has full access to a range of point of care testing pathology, X-rays, CT scan and MRI with an ability to give a range of therapies.
You will provide a clinical review of patients and work collaboratively with an established multi-disciplinary team and senior nursing colleagues.
Keeping frailty patients closer to home where appropriate is a key NHSE aim and is transforming the management of patients outside an acute hospital setting. This is an exciting innovative area of practice but as such is subject to rapid test and learn cycles and is continually evolving. This will suit colleagues who thrive on the ability to affect changes in patient care and like to get involved in new clinical pathways and new ways of working.
