Position: Scheme Manager (Supported Living)
Location: Kingston Upon Hull (Various locations with expectations of travel)
Hours: Full time, 40 hours per week and shift cover may be required when on-call.

About the supported living services you would support
In Swanton Community Care - Hull, we provide personal care and support, for adults with mental health, dementia, learning disability and physical disability support needs. We have two 6-bedroom group living homes, Huckleberry House and Westbridge House along with another 3 services in the community, right in the heart of Hull.
This is an opportunity for an enthusiastic and motivated individual who is ready for a challenging role working across two supported living schemes in Hull. The roles involves supporting three individuals with learning and physical disabilities to move into new tenancies.
Our properties are in a close proximity of each other, We offer a diverse and person-centred services to meet the needs of each individual, we can offer both supported living within a scheme or in individual settings.
We support a diverse set of individuals with varying diagnosis, we work with the local authority to identify and meet the needs of the people we support.
