Job description

This is a post based at North Middlesex Hospital, in which the post holder will be contracted to provide total 7 PA (with 1.25 SPA including an Audit and Governance service role).

At North Middlesex, each consultant will oversee inpatients on a rotational basis, during which period, they are expected to do at least two full ward rounds each week, as well as ensuring early daily review of new patients transferred to the ward and those who need senior input. They will also be expected to participate in daily discharge planning meetings. The post holder will work 16 weeks on the wards spread throughout the year on a rotational basis. They will be supported by the Resident doctors on the Renal team who the post holder will clinically supervise. The consultant will also review patients, supported by the Specialty SpRs, on other medical wards and the Critical Care unit referred with Renal issues such as Acute Kidney Injury, new Nephrotic syndrome, other General Medical issues in Renal transplant or pre dialysis cohorts.

Additional duties consist of 1 Renal diagnostic clinic a week (26 per year) plus administrative time for email queries and clinic correspondence. The clinic does not run when covering the wards.

ON CALL DUTIES

The out of hours (GIM) duties occur 1 in 8 weekends at North Middlesex site between the hours of 8.30 am and 16.30 pm and include chairing the morning ward handover meeting, assessing all patients that move to the medical wards in the Tower block. After 20.00 the consultant will be on call for GIM, off‑site (from home). The postholder will provide only medical cover to the patients on General Medical wards.

This is a well‑established department within the trust and currently we have 3 substantive but part time (shared with RFH) consultants amounting to 1.8 whole time equivalents. We are in the process of expanding our workforce to meet the needs of the local population. Our work centres on in‑patient care for both Renal and General Medical patients based on T8 and Podium 1 wards as well as outpatient activities for Renal Medicine. We also provide an innovative Acute Kidney Injury (AKI) service, concentrating on those patients presenting with or developing stage 3 AKI whilst in hospital. There are 4.5 General Nephrology/diagnostic clinics per week with monthly joint Renal/Diabetes, Renal/HIV, Renal/Urology and Renal/Rheumatology virtual clinics/MDTs.

ADMINISTRATIVE & SECRETARIAL SUPPORT AND FACILITIES

The consultant appointee will have dedicated office space with networked computer access and secretarial support (1 secretary for the Renal department).

A formal job plan will be agreed between the appointee and their clinical manager in line with the Job Planning Guidelines in force in the Trust. Once agreed with the clinical manager, job plans are finally signed off by the Clinical Director. An indicative job plan for this post with indicative Programmed Activities (PAs) is provided below, but this will normally be re‑assessed after 3 months of appointment to ensure it meets the needs of the service.
