Rehabilitation Support Worker Band 3, Merton Enhanced Home Based Rehabilitation Team, Central London Community Healthcare NHS Trust (CLCH) - Merton

We are seeking an enthusiastic Band 3 Rehabilitation Support Worker to join our Community Discharge to Assess service working alongside Physiotherapists and Occupational Therapists.

We are looking for team players with a positive attitude and commitment to providing excellent care and interventions. Our goal is to optimise our patients’ independence and wellbeing, enabling them to return to their own homes following a rapid decline through illness or injury, including falls.

PLEASE NOTE THE FOLLOWING:
  • THIS ROLE IS NOT ELIGIBLE FOR SPONSORSHIP – due to being a fixed‑term contract ending 31st March 2026
  • SHIFT PATTERN – either 8am to 4pm or 2pm to 10pm over a 7‑day period
  • APPLICANTS MUST HOLD a full, valid, UK driving licence and have access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010)
The Rehabilitation Support Worker (RSW) – Responsibilities
  • Works as part of the Multi‑Disciplinary Team to provide a seamless service to a group of patients under the guidance and supervision of an Allied Health Professional.
  • Becomes proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice.
  • Promotes optimum independence and individualised patient‑centred care to a delegated caseload.
  • Supports the team to deliver against key performance indicators and maintain quality of the service.
  • Assists in identification of the health and social care needs, and participates in the delivery and evaluation of care packages developed to meet those needs under direct supervision of a registered health professional.
  • Works as part of an interdisciplinary team within community rehabilitation services to assist in the development of good working relationships with statutory, voluntary and independent organisations.
  • Provides competent and compassionate care to all patients with physical and cognitive difficulties within the community and/or care home settings.
  • Supports Senior Staff by collaborating with colleagues across own and other community teams, acute teams, GPs, social services and the voluntary sector to optimise patient journey, care, wellbeing and outcomes.
